Does the theater bill us for the licensing fee on top of the rental fee?

The licensing fee of a flat $250 - guaranteed by Universal - is on top of the rental of the theatre. The theatre should pay Universal the $250 and then bill you with the rental.

Also if you're using 35mm, there'll be a shipping charge. I think that's been running $50-$75 in most cases.

It depends on where they're shipping from. The first year, we were quoted a price of $125 courier each way, since the films usually come from Toronto. As it happens, we've been very lucky that there has been a copy of the film in town each year, so courier charges have been under $50 for both ways.

You might also luck out like we did. The theater didn't charge us a shipping fee since they had a usual shipment from Universal coming anyway.
I'm not sure exactly how that works, but I wasn't going to question too much. Just took my free shipping and kept my mouth shut.
The most important thing is to try and have a general idea of what you might pay beforehand. Even though that may not end up being the final charge -- as some of the other organizers have reported.

Our theatre didn't charge us shipping either. I think it is just a standard $250 you need to worry about. If concerned, ask upfront. But remind the theatre that this is all for charity, and hope they will be a little understanding.
